* Important: The above time slots are for last names beginning with the
corresponding first letter; no exceptions will be made in order to control
the flow of patients in the best interest of your son/daughter receiving a
proper and thorough examination.

COST? $35.00 (check payable to: SCHS ATHLETICS)

Note: 100% of the monies received for the above physical examinations go
directly back into the SCHS Athletic Department to pay for our on-campus athletic trainer and athletic trainer supplies.

SCHS Athletic Clearance Packet will be available at the Hospital

A PARENT MUST ACCOMPANY STUDENT/ATHLETE TO FILL OUT MEDICAL HISTORY AND SIGN CONSENT FORM BEFORE YOUR SON/DAUGHTER CAN BE EXAMINED.
